No. SAFETY INSTRUCTION TRAINING RECORD FOR LIFTING WORKITPCL CUDDALORE PROJECT SECTION OR DEPARTMENT:Location andWorkEngineerIn chargeName of the sub-contractorDATEATTENDEE(signature)TotalnumberofAttendees :CONTENT AND REQUIREMENTNOTE: This specification is for lifting operation safety procedure, before operation shifting this document should be read familiarly by engineer and informing the labors.1. Personnel working for lifting may not be dressed in loose clothes, and the long hair should be kept within safety helmet, shall not be sloppy.2. During lifting, operation staff should have clear and wide view of the good and work area, or can see the signal clearly from the rigger.3. The operator is prohibited to leave the operation cab when the machine is moving or the material is suspending. Electricity and engine should be shut down and the break switch should be in safety position.4. It is strictly prohibited to break suddenly to control the lifted material from falling.5. It is strictly prohibited to release the material suddenly.6. It is strictly prohibited to pull the heavy objects from the side.7. It is strictly prohibited to operate the machinery that has no sufficient balance weight.8. It is strictly prohibited to operate on unleveled ground.9. Project should unify the signal and other lifting requirements by written distribute and display.10. For the significant lifting and transportation item, it should establish construction plan and safety technical measure in the construction organization design, and apply lifting operation work permit, the construction responsible personnel should guide on site, otherwise it cant allow to work. Lifting operation safety work permit scope and details see Work Permit Procedure.11. The load should be tied firmly. Hook hanging point and load gravity center should be in one line, hook wire rope should be vertical, partial pulling and inclined lifting are strictly prohibited. Falling hook should prevent load touchdown causing sling tilt. When the load is not secured, it is strictly prohibited to open the hook.12. The angle of sling should be not 90 degrees, and maximum 120 degrees. Lifting big or irregular component, it should plug the firm tag line on the load.13. Anybody can not be allowed to stay or cross the lifting work area.14. Lifted load cant stay in the air for a long time. When staying in the air for a shot time, the operators and riggers can not leave the job.15. We should check lifting device and safety equipment before lifting, when the distance between ground and load is 10cm, we should stop and fully checked, after confirmed it is in good condition, then formal lifting.16. Two or more than two crane lifting the same load should comply with the following rules:17. It should distribute load according to the each crane allowed lifting weight when binding.18. The crane should select work radius according to rated lifting ability, it can not allow to over the 80% of rated load, the wire rope must keep vertical in the lifting process, two cranes moving and lifting speed should be the same.19. It should be commanded by experienced rigger in the lifting process, and supervised by professional personnel.20. If there is any machinery accident or deviant phenomenon in the lifting work, we should put down the load, check after the operation stopped, it is strictly prohibited to adjust and repair in the operate. If by fault the crane cant put down the load, we must take the appropriate insurance measures, expect the exclude danger personnel, anyone is not allowed to enter the danger zone.21. Lifting operation should strictly follow the following principle:22. Forbid lifting objects overloaded; 23. Forbid lifting when the signal is not clear or objects of uncertain weight; 24. Prohibit hoisting objects that are not bound firmly or not comply with safety requirement;25. Prohibit lifting objects with a person standing on them or objects with movable structure;26. Never hoist any object when detent or safety device failure;27. It is not permitted to lift when the work place is dark without sufficient lighting; 28. Never hoist oxygen, acetylene cylinders etc. explosive, hazardous objects without safety protective measures; 29. Never hoist sharp-edged objects without protective measures;30. Forbid lifting objects embedded underground or frozen on the ground; 31. Never hoist when the rigger is not qualified. 32. When the wind reached level five in the work place, it can not lifting large wind-engaging area object; When the wind reached level six or above six, it can not lifting.33. When the weather is heavy snow, fog, thunderstorm, or the lighting is insufficient, the rigger can not see the signal of work place or operator clearly, it can not lifting.34. Field which is bearing load is leveled, foundation compacted.35. The machines counterweight is enough, lifting stability is reliable, and meets the technical requirements.36. Horns and the bell signal devices are complete, clear and loud.37. Lifting, walking, slewing, amplitude component has obvious color code and lighting signal.38. View line is clear and in good condition in the cabin.39. Lifting machinery must be marked the maximum load. The brake, limit switch, interlock and protection safety devices of the crane should be complete, sensitive and effective; For the crane which the safety device failure, we must take special protective measures, and set watch man. 40. Overhead crane machines such as tower crane, gantry crane should have reliable thunder-arresting device.41. When the crane is moving on the rail, all the crane must set up a block at the end 2m of rail except the railway crane. Rail should be set grounding device.42. The biggest elevation angle of the boom should not exceed factory standard when the cantilever crane is working, when the factory have no clear standard, the biggest elevation angle should be not more than 78 degree.43. Each component and device of the crane can not be changed or replaced without the permission from Technical Center Department.44. Before using the crane which is free more than one year, through the storm, earthquake, significant accident, new installed, demolition, big repairs or change the important technical performance, we should conduct dead load and live load test according to the factory instruction. When the factory has no standard, we should conduct test according to the following: 45. Lifting Machinery Inspection:Daily inspection before work: the operators should inspect before and after work, and record Lifting Operation Checklist (refers to Mechanical equipment inspection system).46. Regular inspection: Machinery administrator of Project Asset Management Department should check the crane machinery not less than once a month, and solve the identified issue timely. Check the content:47. Safety protective device;Line, cans, containers, valve, pump, hydraulic or pneumatic and other components leakage and work performance;48. Lifting hook, hook nuts and anti- loose device;49. The brake performance and component attrition situation;50. Wire rope attrition and tail fixed situation;51. Chain attrition, deformation and elongation situation;52. Seizing and hanging chain, steel wire and assistive devices.53. Regular inspection: Asset Management Department should organize the regular inspection, Machinery Department should join together, fully inspect all the crane machinery every year, inspection content:54. The metal structure deformation, crack, corrosion and welding seam, rivets, bolt connection, etc.;55. The main component fray, crack, deformation situation, etc.;56. Check the reliability and precision of indicator device;57. Check the power system and controller, etc.58. Crane operation process inspection59. Before operation, the operator should check the lifting requirements according to Lifting Equipment Checklist and it is not lifting if not fit the requirements.60. Lifting Tools61. The management of lifting tools should implement Instrument Safety Procedure.62. Crane operator63. Crane operators should go through professional technical training, they can operate only after passing the safety rules and practical operation examination and getting the certificate.64. Crane operators should be familiar with the rules and relevant knowledge.65. The structure and technical performance of various agencies of crane;66. Familiar with crane operating rules and relevant regulations and laws where the project is located;67. Safety operation requirements;68. Safety and protection device performance;69. Prime mover and electrical basic knowledge;70. Command signal;71. Maintenance and repair basis knowledge.72. Operators should operate according to the signal of rigger, if the signal is not clear or can cause incident, the operator should refuse and notify the rigger. And the operators should obey the dangerous signal which is sent out by any person.73. Large lifting machinery operators, two operators should work at the same time, prohibit to work one by one, it must be one people operate, one people supervise.74. The getting certificate and training work operator should implement Special (Equipment) Operator Procedure.75. Lifting Rigger76. The rigger must go through safety technical training by relevant Country department, only can be allowed to work after passing the examination and getting the certificate.77. ach command large crane hoisting objects, it must use whistle and flag for commanding. Interphone can only be used as the auxiliary tool. It cant use interphone to send out signal directly (except cant use flag, whistle and view line is not sufficient).78. When it is unable to use flag, whistle and the view line is not sufficient, it should use interphone to command signal, and it must ensure that the interphone has sufficient power speaking and listening will be clear, because of the interphone defects or fault, it cant send out correct signal causing accident, we should investigate the responsibility of interphone holder.79. When the rigger cant see the operator and load clearly, it must set intermediate rigger to pass the signal, if found any wrong signal, he should send out stop signal immediately.80. Operators and riggers should closely cooperate with each other, if there is any failure because of buck-passing to each other, it should be punished according to the responsibility. 81. The riggers getting certificate and training work should implement Special (Equipment) Operator Procedure.82. Under age workmen those who below 18 are prohibited to work at site. 83. Workers should not allow working with lungi, dhoti or loose cloth during work.84. Regular training should be conduct for all workmen about the safe work practice & procedures by full time safety officers and maintain the training records85. Horse play not allowed in job, Keep on mind Injury or termination from job or both will be the result of Horse play.86. All work related Personnel protective equipments must be worn as per work nature such as safety goggles for eye protection, Safety helmet for head protection from the falling material hazards, Hand gloves while handling materials, and safety shoes for foot protection.etc.87. Precautions are necessary to prevent from sunburn and to protect against burns from hot materials.88. Always watch your steps are safe while walking, dont run.89. Do not distract the attention of fellow workers. And dont involve in unsafe act which would endanger to others. 90. Sanitation facilities are made for your welfare use it, Forbid the defacing or damaging these facilities.91. A good job is a clean job, and a clean job is the start of a safe job. So keep your working area free from rubbish and debris.92. Do not use the compressor or air blowers to blow to clean your clothes, Hair or hands.93. Never work at height if you are afraid to do so, if you are subject to acrophobia, or if you are nervous or sick. 94. Practice safe manual Handling. Lift the support with legs, not the back. If the load is too heavy, attempt to GET HELP. Keep on mind; approximately twenty percent of all construction related injuries result from improper material handling.95. Nobody but operator shall be allowed to ride on equipment unless proper seating is provided. (Nobody is allowed to operate the equipments unless they are not competent for the safe operation. Display your competency certificate on your equipment or working location.)96. Do not use power tools and equipment until you have been properly trained about the safe work methods and become authorized to use that. 97. Be sure that all safety guards are in place in your tools/Equipment. Do not remove, displace, damage, or destroy any safety device or safeguard furnished or provided for use on the job, not interfere with the use thereof.98. Does not enter to unauthorized area where barricaded. 99. While working around power shovels, trucks, and dozers; be assured the operator can see you and barricade the crane coverage working area to avoid trespassers.100. Never oil, lubricates, or fuel while the equipment is in running or in motion.101. Prior to the maintenance /Servicing of powered tools reenergize, Lock out and tag out from the source of power supply. 102. Barricade danger areas. Guard rails or perimeter cables may be required.(Barricade the endangers area with proper barricading and caution sign board which can able to understand by the workmen.)103. Trenches over five feet deep must be shored or sloped as required. Keep out and be safe from trenches or cuts that have not been properly shored or sloped. 104. Do not stack Excavated or other material near than two feet from the edge of the excavation. Provide cave in protection where required by means of slopping, shoring, shuttering or other protections. Excavations less than 5 ft may also require cave in protection in some instances.105. Use safe ladders, Defective ladders must be properly tagged and removed from service.106. Keep the ladders on firm surface and free from debris, hoses, wires, materials, etc.107. Scaffolds must be constructed according to manufacturers specification and standard code of scaffoldings.108. Assure the scaffolds are erected on firm surface with base plates, Planks on plat forms have securely provided and enough support provided for the stability with sound materials. 109. Use industrial type extension cords and connection should do through ELCB/RCCB. Check the safe earth resistance is available and maintain the earth pits. 110. Wear full body harness while working at height with life line. Fall arrester with lifelines is mandatory while working at unprotected high places. Always keep it as tight as possible.111. Never throw anything from height. This may result injury to someone working below.112. Never enter on a confined space such as Man hole, Shaft, Tunnel or other confined space which having deficiency in oxygen or presence of toxic or flammable gas, or has possibility of engulfed by soils or liquids. Deploy competent person for inspection and assure the safe condition. Adequate measures safety measures and work permit taken prior to work and deploy emergency watchmen on entrance for emergency evacuation.113. Maintain the well-equipped fire points with sufficient number of Fire extinguishers, fire buckets in visible area and trained everyone how to handle it in case of fire emerge
