结直肠神经内分泌肿瘤的治疗进展〔〕:

摘要：神经内分泌肿瘤是一类起源于神经内分泌细胞的异质性肿瘤，其胃肠道多见，根据其是否具有激素分泌功能和有无出现激素引起的临床病症，将神经内分泌瘤分为非功能性〔约占80%〕和功能性〔约占20%〕两大类，而结直肠神经内分泌肿瘤绝大多数为非功能性。随着医疗程度的进步其发病率逐渐升高，目前单一的治疗手段无法使患者得到最大的获益，多学科、多方案的结合治疗具有重要意义，本文就结直肠神经内分泌肿瘤最新治疗进展做一综述。

0引言

神经内分泌肿瘤(NeuroendocrineNeoplasms，NENs)是一类来源于神经内分泌细胞的异质性肿瘤，可发生于全身各个系统、器官及组织，其中以消化道神经内分泌肿瘤最常见，约占所有神经内分泌肿瘤的2/3。其中大局部结肠NENs患者发现时已出现转移，转移性结肠NENs的生存期仅为5个月【1】；直肠NENs大多表现为息肉，总的转移率为2.3%，肿瘤直径大于2cm以及淋巴血管受进犯的直肠NENs更容易发生转移【2】。近年来，由于诊疗技术程度的进步，NENs的发病率在过去30年内增加了5倍【1】。目前国内针对结直肠神经内分泌肿瘤没有形成统一完善的治疗方案，许多治疗缺乏大样本的病例及临床试验，本文就结直肠神经内分泌肿瘤的治疗进展予以概述。

1结直肠神经内分泌肿瘤的治疗进展

由于NENs是一组来源于神经内分泌细胞的异质性肿瘤，具有显著的异质性，其治疗往往需要结合多学科、多方案结合治疗，目的是改善患者的预后，延长患者的生存时间以及进步患者的生活质量。
