15、单表。2、日常巡视制度：质安员每日巡视中，应针对前面分析出的重大危险源部位进行巡视检查，并在安全日记中记录描述危险源受控状态。3、定期检查制度：每周进行一次专项检查，检查结果记录填写施工现场安全隐患排查登记表。4、专项测量监测制度：土坡及坑壁、地面沉陷等按照土方开挖方案需进行仪器测量的。5、预警报告制度：日常巡视、定期检查或专项测量时发现重大危险源出现危险信号时，应书面向项目部报告，情况紧急时，可先口头、电话报告，然后补书面报告。6、危险状态整改消除与验收制度：项目部接到预警报告后应立即组织工程技术、安全技术人员认真研究危险类型、危险程度，采取有效的安全技术措施进行整改，整改施工完毕，应组织评

16、估验收得出是否已消除危险状态的结论。7、应急救援预案制度：项目部编制应急救援预案，当重大危险源失控发生事故时，应立即按程序启动应急救援预案。五、重大危险源危险控制措施1、认真编制各分项技术方案，临时用电方案、“三宝、四口”防护方案，涉及技术数据来源准确，计算方法正确，计算过程无误，确保实体安全。各种方案进行严格的审批审核程序批准后才能实施。2、专项技术方案根据安全生产法律法规须经专家论证审查的，先组织专家审查，根据审查意见进行修改后再形成正式使用方案，确保技术先进可行、安全可靠。3、设备应选用安全可靠性高、有安全生产管理部门颁发的安全技术产品许可证的厂家产品。安装后经检测、调试、验收后才准使用
