冠心病论文:冠心病患者血清尿酸水平与中医病理因素痰、 瘀的相关性研究 【中文摘要】本研究在收集冠心病病例资料基础上对相关数据 进行统计学分析,以期寻找冠心病患者血清尿酸水平与冠心病中医病 理因素“痰” 、 “瘀”的相关性,为冠心病中医辨证提供客观化的依据,从 而更好的指导临床辨证及治疗冠心病。探讨血清尿酸(UA)水平与冠 心病中医病理因素痰、瘀的相关性,为冠心病中医辨证提供客观化的 依据,从而更好的指导临床辨证及治疗冠心病。方法:收集 184 例冠 心病患者的四诊资料,抽血检测血清 UA 水平,采用 2*2 析因分析,探 讨血清 UA 水平与冠心病中医病理因素“痰” 、 “瘀”是否具有相关性,指 导临床对于冠心病的辨证论治。结果:1.将病例分为有“痰” 、无 “痰”组时,UA 水平有“痰”组高于无“痰”组,两组间存在统计学 差异;将病例分为有“瘀” 、无“瘀”组时,UA 水平有“瘀”组高于 无“瘀”组,两组间亦存在统计学差异;将病例按痰、瘀性质不同分 为 4 组:痰浊组、瘀血组、痰瘀互结组、非痰非瘀组,UA 水平痰浊 组瘀血组非痰非瘀组痰瘀互结组,且各组间均存在统计学差异。 将病例按性别分组后发现,男、女病例组 UA 水平均提示有“痰”组 无“痰”组,且组间均存在统计学差异;女性组中,UA 水平在痰浊组 与瘀血组、痰浊组与非痰非瘀组、瘀血组与非痰非瘀组、痰瘀互结 组与非痰非瘀组存在显著差异。以 UA 为应变量,痰浊积分为自变量 作直线回归时发现 UA 水平与痰浊积分存在线性相关。2.以 UA 为应 变量,Cr 为自变量作直线回归时发现 UA 水平与 Cr 水平存在线性相 关。正常范围内的 Cr 水平在有“痰”组无“痰”组;有“瘀”组 无“瘀”患者;分 4 组时发现 Cr 平均水平瘀血组痰浊组痰瘀互结 组非痰非瘀组,但 4 组间无统计学差异。3.TG 水平有“痰”组无 “痰”组,有“瘀”组无“瘀”组。HDL 值非痰非瘀组痰瘀互结组痰 浊组瘀血组,但各组间不存在统计学差异(P0.05)。结论:冠心病 患者血清 UA 水平与中医病理因素痰浊密切相关,且 UA 水平与痰浊症 候积分呈线性正相关。UA 水平与血 Cr 水平存在正线性相关性。血 脂水平与冠心病患者中医病理因素间未发现存在统计学上的相关性, 有待进一步大样本严格控制的研究验证。 【英文摘要】This paper is basically about clinical data collecting on the basis of coronary heart disease,the data were statistically analyzed to search for the correlation between serum uric acid in coronary heart disease and the medicine pathological factors “sputum,” “stasis “, providing an objective basis for the discrimination of Coronary Heart Disease,so as to better guide the clinical discrimination and treatment of coronary heart disease.:To investigate the correlation between the serum uric acid (UA) levels and pathological factors of coronary heart disease associated traditional Chinese medicine, providing an objective basis for the discrimination of Coronary Heart Disease, so as to better guide the clinical discrimination and treatment of coronary heart disease.Methods:184 cases of the four diagnostic information in patients with coronary heart disease,draw blood for the serum UA levels, using 2*2 factorial analysis the correlation of serum UA level and pathologic factors in Coronary Heart Disease “sputum,” “ stasis “ to guide clinical diagnosis and treatment for coronary heart disease.Results:1. The patients were divided into “sputum”, no “sputum” group, UA level of “sputum” group was higher than no “sputum” group, there was significantly different between the two groups; The patients were divided into “stasis”, no “stasis “ group, UA level of “stasis”group was higher than no “stasis “ group, there was significantly different between the two groups also; The case by phlegm, blood stasis of a different nature were divided into 4 groups:Phlegm group, blood stasis group, phlegm and blood stasis group and non-sputum and non-stasis group, UA levels of phlegm group stasis group non-sputum and non-stasis group phlegm and blood stasis group, and there among the groups are statistically significant. The patients were grouped by gender and found that Male and female UA levels in the patient group, “ sputum”group no “sputum”group, and there were significant differences between them; In female group, between Phlegm group and blood stasis group, Phlegm group and non-sputum and non-stasis group, blood stasis group and non-sputum and non-stasis group,phlegm and blood stasis group and non-sputum and non-stasis group, UA levels are statistically significant. The UA as the dependent variable, Phlegm points as independent variables for linear regression found that UA levels and phlegm integral linear correlation.2. UA as the dependent variable, cr as the independent variable for linear regression found that UA levels of linear correlation with Cr levels. Cr levels within the normal range of “phlegm”group no “sputum”group; The “stasis “ group no “stasis “ group;When Divided into 4 groups,found that the average Cr level were Stasis group phlegm group phlegm and blood stasis group non-stasis and non-sputum,but statistics doesnt have found there were statistical differently between them.3.TG levels “sputum”group no “sputum”group, The “stasis “ group no “stasis “ group.The HDL level were non-stasis and non-sputum groupPhlegm and blood stasis groupPhlegm groupstasis group.there were not statisticl differently between them.Conclusion:patients with coronary heart disease, the UA level is closely related to pathological factors phlegm; and UA levels and phlegm syndrome score was positively related. UA levels and elevated levels of serum Cr there is a positive linear relationship. Indicators due to different lipid levels, vary.
