1、Tender Schedule L - Health & Safety and Protection of theEnvironment.1 Health and Safety Statement.l总那么健康和平安的实施将以尊重生命、救死扶伤、维护和保障工程师/业主和中方人 员及劳工健康为宗旨。具备对现场发生事故和紧急事件进行急救和有效诊治,实施疾 病预防控制和保持现场是施工区域的干净和整洁为目的的健康平安保障体系。为实现上述功能和保证上塔马克西水电站土建标的有序进行,上塔马克西水电站 土建工程工程部将成立健康和平安委员会,并在该委员会的领导下,逐步开展和实施 医疗健康计划。我们将配置相应的

2、医疗设施并建立组织保证体系,并根据目前工程实 施进度的实际情况,分步骤对健康平安进行实施。健康平安组织机构图见框图L1。健康平安组织机构图(框图L1)For the purpose of implementing the health and safety policy, the Contractor shall constitute a duly empowered committee within 91 days of the receipt of the Notice to Commence designated as the Health and Safety Committee, w

3、hich shall convene monthly and which shall include:The Medical Officer (or, in his absence, his nominated deputy)The Safety Officer and the Deputy Safety Officer;One representative of the Contractors supervisory staffThree representatives of the labour force representing the various categories of急救站

4、位置及建筑型式(1)本工程在渠首区(A-1区)设一 150m2急救站;在厂房区(C-1)设一275m2 医疗急救站。(2)急救站建筑型式The buildings shall be single storey, mosquito-proof, weatherproof, and secure. the building will be constructed of brick or hollow concrete block walls and partitions and fully thermally insulated. The external walls wi not be less

5、 than 200 mm thick and partitions will be 150 mm thick. External walls will be fair-faced, painted externally and rendered internally. The partitions will be rendered on both faces, except in bathrooms/showers, toilets and other wet rooms where they will be faced with glazed ceramic wall tiling or a

6、pproved alternative up to the ceiling level.The building foundation, skeleton frame and roof will be constructed of reinforced concrete. The roof will be plastered from inside and waterproofed from outside, the waterproofing system including screed to fans, torch-applied waterproof membrane APP 4 mm

7、 thick, and 50 mm gravel separating layer, including all accessories and fixation.The floor will be damp-proofed concrete raised at least 200 mm above the highest adjacent external ground level. The floors will be covered in terrazo tiles. The minimum clear height between floor and ceiling shall be

8、2.6 metres.All walls will be painted with three coats of emulsion paint. All doors will be painted one coat of primer and three coats of hard gloss paint.The building will have fully double-glazed windows each at least 30 percent opening. The windows will be of anodized aluminium and will be so loca

9、ted to provide maximum amount of cross ventilation. Wire anti-mosquito gauze screens will be provided to opening windows, and sun filter curtains will be provided for all windows. The operating theatre and the medicine store will have no windows, the intensive care station, examination rooms and pha

10、rmacy will have appropriately limited fully-sealable windows, and for all other rooms the total window area for each room will be at least 20 percent of the floor area.Each room will have at least one door and each door shall be provided with a three lever lock and three keys. In addition, three mas

11、terkeys will be provided. All internal doors will be of skeleton framed flush softwood, 4 mm plywood faced of a minimum size 900 mm x 2100 mm. The operating theatre door will be provided with hermetic sealing. External doors will be made of two-skin 4 mm pressed steel with necessary bracing and fram

12、es. The steel will be painted with 2 coats of zinc oxide primer and 2 coats of aluminium paint.The building will be well ventilated and provided with suitable heating units and insulation as necessary. Heating and ventilation will be rated such that a temperature of 21 can be maintained at all times

13、. Lighting will be of the fluorescent tube type. All rooms2will have one switched electric socket per 5 m floor area or part thereof, with a minimum of two sockets.We will provide for the building running hot and cold treated potable water, uninterrupted electricity, and a sewage system, and will be

14、 regularly cleaned and maintained throughout the duration of its use.急救站职责、提供的服务及药品设施配置The Contractor shall be responsible for carrying out the following general activities at the Clinic / First Aid Station in order to provide the standard of general medical service required:implementing appropriate

15、 procedures in relation to cleaning, disinfection, inspection, packaging, sterilisation, transportation and storage of reusable medical devices;ensuring that reusable medical devices are handled safely and decontaminated effectively prior to re-use;establishing robust and checkable systems for the o

16、rdering, recording, handling, safe keeping, safe administration and disposal of medicines used in relation to the provision of the Medical Service;making arrangements to minimise the risk of infection and toxic conditions and the spread of infection between Patients and Staff;establishing a clear pr

17、escription request process;undertaking medical reviews of patients at clinically appropriate time intervals, to be recorded in the patients medical notes.The Clinic / First Aid Station will also be at all times properly supplied with appropriate quantities of all relevant types of medicine. As a min

18、imum the facilities shallinclude the following fully equipped facilities:operating theatre for minor operations and for the use of visiting emergency surgical staff in case a patient requiring an urgent operation cannot be safely moved for referral;intensive care station containing no less than 3 No

19、. equipped bed units;recovery station containing not less than 4 No. partition rooms each containing 2 No. beds;laboratory with climate-controlled sample storage;patient bathroom with bath and shower facilities;patient toilets (two male, one female);laundry with bedding store;patient catering kitche

20、n;not less than 2 No. examination rooms;reception and waiting room;securely lockable pharmacy;secure climate-controlled medicine storeroom;miscellaneous medical supplies storerooms;1 No. office room for each doctor;nurses office room with sufficient work places;staff recreation room;staff kitchenett

21、e;staff toilets (one male, one female);general storerooms.We will be responsible for maintaining direct voice communication from anywhere within the Site to the Clinic / First Aid Station, either by telephone connection or by Site radiotelephone communication system.The general medical service to be

22、 provided by the Clinic / First Aid Station shall include, but not be limited to, the following:on-site clinical emergencies, basic resuscitation, urgent and emergency referrals to an identified hospital in Kathmanduimmediate/necessary/emergency treatment;wound management and suture removal;minor su

23、rgery proceduresvaccinations and immunisations;management of patients who are ill or believe themselves to be ill, with conditions from which recovery is generally expected within less than one week, for the duration of that condition, including relevant health promotion advice and referral as appro

24、priate;home visits to Project accommodation on Site when clinically required in line;L1.3.5 First Aid Provisions on SiteWe shall provide and maintain in ready condition boxes of first aid materials together with stretchers at prominently marked locations within 100 metres of each working site on the

25、 surface and underground or as required by the Engineer. The contents of each first aid box shall include but not be limited to:wound cleaner (500 ml)6 roller bandages (80 mm x 5 m)swabs for cleaning wounds6 roller bandages (100 mm x 5 m)cotton wool for padding (250 g)2 rolls elastic adhesive (25 mm

27、e eyewasheyepads3 pairs disposable glovesL1.3.6 First Aid TrainingWe shall, through the auspices of the Health and Safety Committee, institute and operate a first aid training programme to ensure that each foreman or work crew leader in control of 20 or more workmen is trained in first aid and posse

28、sses a valid certificate to that effect issued by the Red Cross Society of Nepal, or equivalent qualification, within 3 months of his appointment. All First Aid qualified personnel shall be identified to the Engineer and will be issued with green and white stickers to be worn on their hard hats.Ll.3

29、.7 AmbulanceWe will provide one four-wheel-drive ambulance vehicle to the approval of the Engineer, available to provide an ambulance service on a 24-hour basis continuously at all times when work is in progress including within the site establishment areas, for transportation of patients from any l

30、ocation associated with the Works to the Clinic / First Aid Station and, on referral of a patient not requiring helicopter evacuation to an identified hospital in Kathmandu.The ambulance will be manned at all times on a shift basis by a driver and trained first-aider full-time. The first aider will

31、be trained in basic First Aid and certified as such by the Nepalese Red Cross Society or approved equivalent.The ambulance vehicle will be capable of transporting two stretchered patients while containing additionally equipment such as portable stretchers, monitors, oxygen set and cylinders etc.The

32、ambulance will at all times be stationed at the Clinic / First Aid Station, except when engaged on cases of medical emergency or referral.The ambulance will be fitted with telecommunications equipment in order to be fully incorporated within the Site radiotelephone communications system / First Aid

33、Station and the referral hospital in Kathmandu.We will be solely responsible for ensuring that the ambulance is at all times stationed in the specified location and is available for immediate use, except when it is already engaged in a medical emergency. If in the case of a medical emergency the Emp

34、loyer is unable to obtain immediately ours assistance for any reason, the Employer shall have the authority to give direct instructions for the use of the ambulance and such instructions shall be carried out forthwith by ours medical staff and ambulance crew.Ll.3.8 Helicopter EvacuationEvacuation an

35、d/or referral shall be at the sole discretion of the Contractors Medical Officer, which may not be delegated and must be effected by the most direct and expeditious route and method; given the prevailing weather conditions and time of the emergency be it by ambulance or helicopter.We will have an ag

36、reement with at least two of the helicopter companies operating in Kathmandu for quick evacuation in case of serious accidents to ensure that at least one of them will be available on a 24-hour basis for evacuation from the Site.The type of helicopter to be made available at all times for medical ev

37、acuation will be a MBB/Kawasaki BK 117 or equivalent, permitting a minimum of two patients to be easily transported and equipped to allow space for patients and medical personnel and to accommodate the necessary amounts and types of medical equipment. The medical equipment that is to be available on

38、 the medical evacuation helicopter will include basic emergency monitoring units as well as more sophisticated instruments for maintaining burn patients.A heli-pad eill be provided by our near to the Clinic / First Aid Station with the minimum dimensions of 30 m x 30 m. The heli-pad w川 be constructe

39、d with an all weather hard cover, including appropriate marking and drainage.L.2环境保护我们针对本工程的情况和环境要求,工程部在进场的同时,指定具有专业水准的、 审查合格的专人负责环保工作,建立环境保证体系,保护总体目标的实现。我方在编报施工组织设计文件的同时,编制一份施工区和生活区的环境保护措施 计划,报送监理工程师审批。其具体内容包括:(1)砂石料加工系统废水处理;(2)混凝土拌和系统废水处理;(3)与本标有关生活营地区的生活污水处理;(4)本标范围内的大气环境保护;(5)本标范围内的声环境保护;(6)本标施工

44、理及汽修等产生的生产垃圾含有较多的金属类废品,其中局部仍具 有一定回收价值的,尽可能回收利用处理。其它生产垃圾统一运至垃圾填埋场进行填埋处理。(3)施工弃渣按招标文件的有关规定和监理人的指示做好施工弃渣(土)的处理,严格按指 定的渣场弃渣,并采取碾压、挡护等措施,不任意堆放弃渣,严禁向河道乱弃渣,防 止和减少水土流失。对于难以防止而滑入河道的渣土、因施工造成场地塌滑、毁坏林草和场地造 成泥沙漫流等问题,接受发包人、监理人及水行政主管部门的监督检查,并及时、无 条件地进行处理。(4)有害和危险品按规定,对有毒有害和危险品严格管理,防止污染事故和平安事故的发生。L2.3 废液处理:生活污水处理措施

45、在生活区内设置临时污水沉淀池、化粪池、拦污栅,地面设置砖砌排水沟,地下 埋设排污管,使排污管道与已建的生活污水主管道相接的排污管不得有垃圾和未经处 理的粪便排入,同时实行雨污分流。对于零散的施工活动地其生活污水,能接到污水 处理系统的,接到系统进行处理,不能接的修建化粪池,处理后与当地农民签订合同, 定期清运,用作农家肥。生活污水经场内过滤沉淀池处理达标后,集中排至监理工程 师指定地点。不将生活污水直接排入澜沧江水体。生产废水处理措施工程生产用水主要包括砂石料冲洗用水、混凝土浇筑用水、混凝土养护用水、混 凝土冷却水、汽车、机械冲洗用水和消防用水等生产性建设用水。生产废水含泥量高,污染物主要为悬

46、浮物,基本不含有毒物质,但直接排放对生 态环境影响较大。因此,各作业面的生产废水通过排水沟、排水泵等设施汇集到集水 池,经处理后尽可能循环使用。沉淀池定期清理,统一运至弃渣场。(1)砂石料冲洗废水处理考虑到砂石料冲洗废水量大、且禁止排入河道,因此,设计采用回用水处理系统 对砂石料冲洗废水进行处理。该系统设置在砂石加工区,任务是收集砂石料筛分冲洗 废水,并进行处理达标,以期回用。处理方法采用平流式沉砂池和斜管沉淀池两级处 理。主要构筑物包括废水调节池内、平流式沉砂池、机械反响池加斜管沉淀池、一座 清水池和加压泵房等。污泥通过自然风干后挖除填埋,干化场中的废水经管道收集, 自流送至回水处理系统中的

47、废水调节池内。(2)混凝土拌和系统废水处理在每个混凝土拌和站废水排出口下游,设置矩形平流式沉淀池,沉淀池(尺寸:3.0mx4mxl.55m)分二格,沉淀池的两格轮流使用,沉淀下来的泥浆定期人工清理。泥浆晒干脱水后与工程弃料一并处理,上清液回用于工地洒水降尘。(3)机修及汽修系统废水处理汽车保养的清洗工艺,包括用热水清洗、或用热水清洗完后再用冷水冲洗两种形 式。根据清洗工艺不同,汽车保养厂产生的废水可按碱性乳化含油废液和碱性乳化废 水考虑。碱性乳化含油废液处理采用“废液汇集一废液隔油池一处理槽一废水 隔油池”工艺流程,碱性乳化含油废水处理可采用“废水汇集一废水隔油池一处理 槽一回用或排放”工艺流

48、程。淤泥清理后运往渣场,沉淀后的废水,除自然挥发外, 其余废水仅可作为施工临时道路喷洒降尘。现场所有部位使用的卫生设备、污水处理系统和设施的详细资料见(武晓杰)L2.4污染预防施工区粉尘污染的预防(1)密切配合业主和工程师对其施工设备、除尘装置和拟采用的施工方法等进 行检查和审核。(2)施工期间,除尘设备与生产设备同时运行,并保持良好运行状态。(3)选用低尘工艺,钻孔要安装除尘装置。(4)混凝土系统配置除尘装置,定期检查除尘装置的运行情况,及时更换和修 理无法运行的除尘设备。(5)尽量防止将易产尘物料储存或堆放在敏感受体附近。(6)施工场地内限制卡车、推土机车速以减少扬尘。(7)经常清扫施工场

49、地,保持场地的清洁,并充分地向多尘工地洒水,防止施 工场地及机动车在运行过程中产生扬尘。道路每天至少洒水四次,施工现场每天至少 洒水两次,在公路两旁有条件的地方进行绿化以及尽可能采用密封运输等方式控制与 削减交通粉尘。(8)采用闭路循环系统,控制与削减砂石料及混凝土拌和系统粉尘。(9)散装水泥、粉煤灰由封闭系统从罐车卸载到水泥储存罐,所有出口配有袋 式过滤器。(10)用以运输可能产生粉尘物料的敞蓬运输车,其车厢两侧及尾部均配备挡板,可能产生粉尘物料的堆放高度不得高于挡板,并用干净的雨布加以遮盖。噪声污染预防工程施工产生的固定连续噪声源主要有砂石骨料加工系统,其余的爆破、施工机 械噪声为间歇瞬时

50、噪声,以及运输产生的交通噪声,相关要求如下:(1)根据准备使用的施工或运输机械设备的类型、施工方法,制定降低噪声的 方法和措施提交工程师审查、批准。(2)密切配合工程师对降噪措施进行检查和检测。(3)施工期间除按上述标准控制施工场地噪声外,禁止任何持续的高强噪声的 操作。(4)在制定施工计划、施工方法及降噪措施时,充分考虑噪声对周边其它环境 敏感点的影响,委派环保专职人员监督实施,使施工场界和敏感受体的噪声水平能达 到噪声控制标准,并且确保以下措施的实施:施工期间,将动力机械设备合理分布在施工场地,尽量防止在敏感受体附近 同时布置或运行多套动力机械设备。施工期间,于施工场地与周边地区和敏感受体

51、之间合理安装声障设施,以有 效阻隔噪声向施工场地周边和敏感受体的方向传播。采用的声障设施要设计合理、性 能优良、坚固耐用。通过选用符合相关标准的施工机具、加强设备的维修和保养、防止夜间施工 等措施,设立标志牌等方式减少噪声;对必要的施工场所进行防护。加强设备的维护和保养。各种动力机械设备暂时不用时关机。混凝土生产系统的空压机设置消声器。振动大的机械设备使用减振机座降低 噪声。严禁在施工场界内使用气喇叭。采取必要的预防措施保障职工的听力健康。渣料堆放和储存(1)在倾倒和开挖任何材料之前剥离储存地表土以备后用。地表土根据种类不 同按不同区域分开储存。开挖材料和地表土单独存放在指定的区域,严禁在开挖

52、点附 近随意堆放。(2)后期工程不使用的开挖料在图纸指定或工程师选定的区域处理。指定渣料 堆放区的地表土事先剥离并在最后替换为现有地表土。然后根据招标文件要求在工程 师指导下在地面种植草坪或灌木。workmen and labourers.L1.2健康平安委员会职责(1)制定、显示、并宣传恰当的平安和应急条例来防止涉及到如果发生火、煤 气、交通、工程操作的意外事件;(2)设立并维护所有临时的围墙、挡墙、栅栏、信号和灯。信号包括标准道路 信号、报警信号、危险信号、控制信号和指示信号以及特定的平安信号。所有信号采 用英语和尼泊尔语,并且清晰易读,得到监理工程师的批准。工程进行期间保持这些 信号干净

53、、清晰;(3)采用尼泊尔语和英语写的通知必须设立在公众可能看的的所有地点,从而 可以警示在工程里的群众。(4)按工程师的决定,在工程开工时免费发放给所有的工人以下物品:合适的衣服、鞋类、平安帽、头灯、雨衣、平安眼镜、护耳器、防尘面罩、手套、 护目镜、平安带、自我救援装置、氧气呼吸器等。在所有工人进入工作区或者相关指 定区域前,对其如何使用这些设备进行培训。必要时免费提供所有这种设备给现场监 理工程师和雇主的员工。(5)按规定提供给每一个工作岗位担架、急救药箱和救援设备;(6)提供和维护所有的临时性跳板、梯子、脚手架和盖子来保护防止现场的高 空坠物和残片,使用完毕移走这种跳板、梯子、脚手架、和保

54、护装置;(7)提供足够的照明设备及其备用设备,保证现场照明要求;(8)提供并保养平安有效的起重设备,包括绳索、吊索、滑轮和其他起重滑车。 每一设备有一个合适的有切实根据的的测试证明;(9)提供足够的脱水和备用抽水设备;(10)确保去工程现场的通道的平安畅通;(11)为员工、以及监理工程师、雇主和雇主的其他承包人的现场员工提供所有 要求的医疗和急救服务。L1.3健康和平安计划(1)在开工日期后56天内,在提交健康和平安规章基础之上,起草现场健康安 全计划,并得到监理工程师的同意。(2)健康和平安计划执行的业务法规业务法规基于一个认可标准,超过国际劳工组织(日内瓦)施工工程健康和平安(3)渣料堆放

55、区分为矿物堆放区、岩石料区和适于植物生长的土壤区。垃圾堆放在废石区。(4)在征得工程师同意和相关部门批准的情况下对垃圾进行处理。(5)渣料进行最大限度的压实以方便运输(6)边坡成形并保持稳定,对边坡进行4米宽的压实以便特殊渣料的堆放。边坡 由石料组成并与地面进行适当过度。(7)紧贴着渣料堆放区筑造一条土堤以遮挡冲刷掉的较好的石料。有效维护排 水和堤岸直到植物成长完成。野生动物和鱼类的保护采取可行措施防止对工地附近动物和鱼类的影响。保证员工在现场或其附近地域 不从事任何动物和鸟类的非法打猎、射击、诱捕或收集活动,也不从事任何非法的钓 鱼等活动让员工知晓其在施工期间可能遇到的濒危保护动物。任何违反

56、上述规定的员工马上解雇。L2.5对紧急状况的反映应对污染事件包括任何危险物质泄漏紧急反映的设备、系统和培训。周边人员的紧急疏散、撤离(1) 一般原那么人员紧急疏散、撤离,迅速将警戒区及污染区内与事故应急处理无关的人员撤离, 以减少不必要的人员伤亡。(2)事故现场人员清点,撤离的方式、方法迅速撤离事故现场人员及泄漏污染区人员至上风处,并立即进行事故现场隔 离。需要佩戴个体防护用品或采用简易有效的防护措施,并有相应的监护措施。明确专人引导和护送疏散人员到平安区,并在疏散或撤离的路线上设立警戒 人员,引导和监护此事的实施。(3)非事故现场人员清点,撤离的方式、方法清点非事故现场人员,并迅速将其撤离泄

57、漏污染区,同时进行事故现场隔离。明确专人引导和护送疏散人员到平安区,并在疏散或撤离的路线上设置疏导 及警戒人员。应急救援培训(1)将应急救援培训纳入年度平安教育培训计划。(2)培训的人员包括全厂员工,重点人员是“应急救援领导小组”、“应急救援小 组”、一线生产员工等。(3)根据预案和事故可能发生的对策措施编写培训教材。(4)培训内容是在突发情况下,接报警程序、现场自我保护和应急处理。(5)将应急救援培训纳入新工人上岗培训内容。对工人进行紧急事态时的接、 报警培训,包括接、报警程序与接、报警内容。应急救援演练在不影响正常生产的前提下,计划每年安排演练一次。L2.6现场修复在工程完工后,清理其所有

58、的如下临时建筑:办公室、住所、仓库和存放区、水 泥板、厚板、油罐、管道、围墙、地下结构或其他人为材料,把施工区域恢复到以前 的状态,耕地恢复到最初的生产能力。废物、金属和混泥土残骸、柴油和沥青等污染土壤的移除处理、污水消毒系统、 按照工程师认可的深度进行翻地(根据土壤自然环境决定)、并重新撒布以前的地表 土、最后使之美观。假设土地收归工程所有,那么承包商应在工程师指导下恢复并种植适合的植被。这项 工作在工程师和业主的规定下尽可能快的进行,在合同结束时不能有所遗漏。在需恢复到初始状态的区域实施包括种植草皮、花圃恢复、种植树木和灌木等工 作。L2.7 Community RelationsIn t

59、he location and operation of our equipment and facilities and in the execution of the Works the Contractor shall at all times bear in mind and to the extent practicable minimise the impact of our activities on local communities. To this end, we shall liaise with the local government authorities on r

60、elevant matters. Any problem which cannot be resolved by the Contractor shall be referred to the Engineer.
